Children cannot be enrolled without proof of current, age appropriate immunizations.

You must provide written proof of additional immunizations as they are administered. We will notify you if we have not received this proof, and, as prescribed by our licensing authority, you will have 15 days to provide written evidence that the immunizations were administered.

If updated documentation is not provided, your child will be excluded from the program. If your child is not immunized because of religious or health reasons, you must provide either a signed affidavit from the child’s health care provider or a statement, signed by a child's parent, that the child is being raised in a religion whose teachings are in opposition to immunization.
